At the end of Term 3, information was sent home regarding to a new health and safety application being used by the Pacific Sports Department called CareMonkey.

What is CareMonkey?

  • An innovative, parent-controlled electronic medical form for schools and sporting groups with a duty of care.
  • An electronic version of the paper-based consent forms currently used.
  • An online avenue providing parents with the opportunity to update medical information promptly, regularly and accurately.
  • An application that allows our teachers/coaches immediate access to the emergency information provided.
  • An application that allows access online and offline from all of our game and sports event locations.

Over the holidays an email from the CareMonkey system was sent to parents inviting you to enter the details for your child in an electronic medical form. Parents are asked to please check their emails and complete the required information. Reminder emails from this system will be sent in due course to those who have not completed the process.

If you have any concerns with this application or your profile set up, please contact our Sports Aide, Mrs Natalie Campbell, directly at: ncampbell@pacluth.qld.edu.au. Thank you to the many families who have already completed this process.
